Michael Roffelsen, MS | Chief Executive Officer
Mike was appointed Chief Executive Officer at Green Bay Rehabilitation Hospital in April 2022.
Prior to joining Green Bay Rehabilitation Hospital, Mike worked in South Florida as the Chief Executive Officer in another rehabilitation hospital.
Mike has over thirty-five years of experience with numerous healthcare organizations in a variety of leadership roles including marketing, business development and operations.
Mike graduated from Indiana Wesleyan University with a Master of Science degree in Management. He earned his Bachelor of Art degree in Communications from Concordia University Wisconsin.
Matt Luders, MBA | Chief Operating Officer
Matt Luders is a healthcare leader with nearly 15 years of experience advancing patient care, operations, and community partnerships across Northeastern Wisconsin. A Door County resident the past 25 years, he began his career at Door County Medical Center, where he discovered his passion for connecting people with the resources that support better health.
He later held operational leadership roles within the cancer service lines at HSHS and ThedaCare, experiences that strengthened his commitment to collaboration and innovation in patient care. In 2022, Matt joined Green Bay Rehabilitation Hospital prior to its opening as Director of Marketing & Business Development, helping establish a hospital focused on restoring function and independence for patients throughout the region.
Matt holds an MBA in Organizational Leadership from the University of Upper Iowa and is certified in Lean Six Sigma. He brings a continuous-improvement mindset to leadership and is driven by supporting both team and community success.
Outside of work, Matt enjoys the outdoors with his family in Door County and serves on the boards of Curative Connections and the Altrusa Hospitality House in Green Bay.
Kim VandeYacht, PharmD, RPh | Director of Pharmacy
Kim brings to Green Bay Rehabilitation Hospital 15 years of experience as a clinical pharmacist. Kim attended Purdue University graduating with a degree in Business Management before deciding to pursue a career in pharmacy where she then went to attend Creighton University’s School for Pharmacy. Kim has previously been a clinical pharmacist in a number of settings including home infusion, behavioral health, cancer & specialty pharmacy, and hospital acute care pharmacy.
When asked why she chose GBRH, Kim shared that she really liked the concept that the patient gets full-service care from intense physical rehab to medical management including dietary, nursing, speech, and in-patient pharmacy services.
In Kim’s free time she enjoys reading a good murder mystery and spending time with my family and friends traveling, boating, attending Packer games, and watching her kids in their extra-curricular activities.
Lili Wagner, RN | Director of Patient Outcomes
Lili Wagner is a Registered Nurse with over 20 years of experience in acute care, specializing in medical-surgical nursing, orthopedic care, and pre/post-operative recovery. She earned her Bachelor of Science in Nursing from Regis University in Denver in 2004 and has since built a diverse career that includes bedside care, case management in LTACH and SNF settings, and leadership in patient outcomes.
Currently serving as the Director of Patient Outcomes at Green Bay Rehabilitation Hospital, Lili is committed to ensuring patients receive the right level of care at the right time. Though no longer at the bedside, she continues to advocate fiercely for patient access to high-quality inpatient rehabilitation—believing strongly in the healing power of compassionate, individualized care.
Guided by the philosophy of treating every patient like family and inspired by Ernest Health’s core value, “we are first and foremost passionate caregivers,” Lili brings heart, insight, and decades of clinical knowledge to her leadership role.
When she’s not working, Lili enjoys life in beautiful Door County with her husband, two children, and a beloved (and ever-growing) crew of animals. You’ll often find her on the water boating and fishing, or exploring new destinations with her family.
Heather Tillman, RN | Director of Quality
Heather brings to Green Bay Rehabilitation Hospital, over 20 years of healthcare experience, many as a Registered Nurse. Previously Heather has worked in leadership roles in infection prevention, staff development, ER, and post-acute care environments.
Heather is a staunch believer in acute rehab and the care that is provided in the inpatient rehab hospital setting. Heather joined GBRH because she believes that it is not only important now but will only continue to grow because the purpose is to get patients back home. What Heather loves most about her role is the attention that she gets to give to policy & procedure development, after all, it is in large part what we do to ensure the best possible care is given to patients. In Heather’s free time, she enjoys camping, watching a good show and shopping.
Dr. Elizabeth Polacheck, MD | Medical Director
Dr. Elizabeth Polacheck, MD joined Green Bay Rehabilitation Hospital in May of 2024 and serves as Medical Director. Dr. Polacheck is a physical medicine & rehabilitation physician with nearly 40 years of service to patient care. Dr. Polacheck completed medical school at the University of Wisconsin-Milwaukee, Medical College of Wisconsin and her residency at the University of Pittsburgh; her senior year as Chief Administrative Resident.
Dr. Polacheck’s past service includes both inpatient and outpatient rehab at Aurora St. Lukes in Milwaukee along with serving as Medical Director of the West Allis Memorial Hospital’s rehab unit. Dr. Polacheck has given specialized attention to EMGs/nerve conduction studies, IMEs, and disability evaluations.
When asked, Dr. Polacheck chose Green Bay Rehabilitation Hospital to continue her practice because she could “tell right away what a wonderful place this and all the people are.” In her free time, you will find Dr. Polacheck spending time with family, who live in Door County, now that she lives so close to them again. Volunteering is also important, where she was involved with, You are Special, an organization that partners with Milwaukee area organizations to provide birthday cake to those with drug addiction, victims of domestic abuse and homeless teens. Fun fact, Dr. Polacheck’s two daughters, Katie and Claire, were both in the Peace Corps, something that makes her very proud!
Tracey Fuller, MSN, BSN, RN | Chief Nursing Officer
Tracey Fuller serves as the Chief Nursing Officer at Green Bay Rehabilitation Hospital, bringing over 22 years of nursing experience spanning clinical practice and healthcare leadership. She holds a Bachelor of Science in Nursing from Abilene Christian University and a Master of Science in Nursing from the University of Phoenix.
Throughout her career, Tracey has held key leadership roles, including Director of Nursing in acute care settings, where she led high-performing teams and spearheaded quality improvement initiatives. Her clinical background includes emergency care, trauma ICU, and the cardiac catheterization lab—giving her a deep, firsthand understanding of patient care from bedside to boardroom.
Tracey was drawn to Green Bay Rehabilitation Hospital for its mission-driven approach and unwavering commitment to healing and restoring quality of life. As a leader, she is passionate about fostering a culture of excellence, where nurses are empowered, patients feel truly cared for, and every team member contributes to meaningful outcomes.
Outside of her professional role, Tracey is a proud wife and mother of three. She enjoys organizing community empowerment events, supporting her children in their athletic and cheer pursuits, and finding creative ways to uplift and inspire those around her.
Zach Vandenberg, DPT | Director of Therapy Outcomes
Zach Vandenberg, DPT, is the Director of Therapy Outcomes at Green Bay Rehabilitation Hospital, bringing over 10 years of experience in healthcare across a wide range of clinical and leadership settings. He earned his Honors Bachelor of Biomedical Sciences in 2015 and his Doctorate of Physical Therapy in 2017 from Marquette University in Milwaukee.
Throughout his career, Zach has served as a Physical Therapist in multiple care settings, Director of Rehabilitation within a skilled nursing facility, and Multi-Site Clinic Manager with Athletico Physical Therapy. These roles have shaped his strong foundation in clinical excellence, operational leadership, and outcomes-driven care.
At Green Bay Rehabilitation Hospital, Zach is passionate about working within an interdisciplinary team to deliver robust, evidence-based therapy services to the community. He is deeply motivated by the opportunity to make a meaningful difference in the lives of patients and their families and values being part of an organization that prioritizes doing what is best for every patient.
Outside of work, Zach enjoys staying active through yoga, hiking, and biking, as well as playing guitar, gardening, and spending time with his family. He is also actively involved in the community, serving on the board of Tayo Clinic, which provides mental health services to uninsured individuals across Wisconsin.
Stephanie Banner | Director of Human Resources
Stephanie Banner is the Director of Human Resources at Green Bay Rehabilitation Hospital, bringing over 15 years of leadership experience in healthcare. With a career grounded in a passion for supporting those who deliver direct patient care, Stephanie has held a variety of progressive roles, including Patient Access Manager in the Emergency Department, Human Resources Supervisor, and Human Resources Manager.
Stephanie holds a Bachelor of Science in Education from Texas State University. Her commitment to building strong, supportive workplace cultures has guided her approach throughout her career, especially in the fast-paced, high-pressure environment of healthcare. She finds great reward in attracting, developing, and retaining the dedicated professionals who provide exceptional care to patients.
At Green Bay Rehabilitation Hospital, Stephanie was drawn by the opportunity to influence the employee experience and champion a work environment where caregivers can thrive and patients benefit from consistent, compassionate service.
Outside of work, Stephanie enjoys reading suspense novels, spending quality time with her family, and managing a retail shop she co-owns on the scenic Chain of Lakes. She also actively supports her local community through various initiatives.
Jake Koeshall, MHA, LNHA | Director of Marketing and Business Development
Jake Koeshall serves as the Director of Marketing and Business Development at Green Bay Rehabilitation Hospital, bringing more than 12 years of healthcare experience across acute care, post-acute care, behavioral health, and healthcare administration.
Jake holds a Master of Healthcare Administration, along with a Bachelor’s degree in Biomedical Sciences and a minor in Nutrition. His diverse professional background provides him with a well-rounded understanding of the healthcare continuum, from frontline patient access to senior leadership roles. Prior to joining Green Bay Rehabilitation Hospital, Jake held positions including Patient Access Representative II at Ministry St. Michael’s Emergency Department, Treatment Specialist at Winnebago Mental Health with the Wisconsin Department of Health Services, Admission Specialist at ThedaCare – Peabody Manor, and Nursing Home Administrator at ThedaCare – Peabody Manor.
Jake was drawn to Green Bay Rehabilitation Hospital because of its strong reputation for exceptional patient outcomes and meaningful community impact. Encouraged by a current employee to apply, Jake quickly recognized the organization’s commitment to patient-centered care after meeting the leadership team, touring the facility, and witnessing the quality of care firsthand. He was particularly inspired by the leadership team’s adherence to strong guiding principles and their shared dedication to improving healthcare within the community—values that closely align with his own professional mission.
Outside of work, Jake enjoys spending time with his wife, their three-year-old daughter, and their dog. As a family, they love camping, boating, golfing, fishing, watching Green Bay Packers games, and seeking new adventures together. While not currently affiliated with a formal volunteer organization, Jake remains actively involved in supporting various post-acute healthcare programs and community events, reinforcing his ongoing commitment to serving the local community.
Rick Koebel | Director of Facilities Management
Rick Koebel is the Director of Facilities Management at Green Bay Rehabilitation Hospital, where he leads maintenance operations and facility strategy with a focus on safety, compliance, and performance in a healthcare setting. Though newer to the healthcare industry, Rick brings more than 30 years of experience in facilities maintenance and management across manufacturing and energy sectors.
Before joining Green Bay Rehabilitation Hospital, Rick held leadership roles at companies including Sodexo (Procter & Gamble), BigOx Energy Corporate, Tufco, and KI. In these positions, he managed complex facilities systems, oversaw cross-functional maintenance teams, and led continuous improvement initiatives to enhance operational efficiency and reliability.
Rick earned his Bachelor of Science in Business Management from the University of Wisconsin–Stout, and holds technical training in Maintenance and Welding from Northeast Wisconsin Technical College. His move into healthcare was driven by the opportunity to take on new challenges in a purpose-driven environment, supported by a strong leadership team and a culture that values work-life balance.
Outside of work, Rick enjoys boating, jet skiing, catching movies, tackling home projects, and repairing small equipment. He values time spent with family and friends and actively supports his local community.
